Medical Cannabis Follow-up = $150

Hello There! It must be time to renew your cannabis card!

The Utah Medical Cannabis Act requires patients to be seen 2 times per year, but some patients may be seen more frequently.  

If you are due for a follow-up evaluation, please complete the form below.

Most follow-up appointments will be done via Tele-medicine using Facetime or Google Duo

Once submitted, our schedulers will contact you to schedule.

Please allow 3-5 business days for response.

Follow-Up Form

  • First Card is good for 6 Months

  • Second Card is good for 6 Months

  • Third Card is good for 6 Months

  • 4th card and every card after can be renewed for 1 year - Per your provider

Medical Cannabis Card Renewal Steps

  • Enter evs.utah.gov into an internet browser
  • Login with UtahID/Email and Password (see above)
  • Go to TRACKING INBOX (top left)
  • Click PATIENT, then PATIENT again (DO NOT CLICK NEW PATIENT)
  • Click the line with your name on it
  • Edit info if needed (check ID expiration date)
  • Scroll to the very bottom left, then click SAVE AND SUBMIT FOR CERTIFICATION
  • Status needs to state “Awaiting Certification”

Payment Steps

Must Turn off pop-up blockers
  • Visit EVS.UTAH.GOV
  • Login with Utah ID / Email and Password
  • Go to TRACKING INBOX (top left)
  • Click PATIENT, then PATIENT again
    • (DO NOT CLICK NEW PATIENT)
  • Click the line with your name on it
  • Click PAYMENT tab
  • Click +NEW
  • CLICK HERE TO PAY
    • (Pop-up block may need to be turned off to finalize payment)
  •         Enter card info and submit
